The Role:
The aim of a Foundation Health Coach is to work as part of our Prevention Coaching team to deliver excellent patient care across our National Diabetes Prevention Programme (NDPP), Diabetes Structured Education (DSE) programme and our Tier 2 Weight Management programme.
As a Foundation Health Coach, you will provide high quality, evidenced based clinical care in line with Oviva training and protocol, and professional standards
You will adopt an efficient, patient-centred approach to care responding to individual patients needs, as well as working as a team to meet the service referral requirement
You will work to meet Oviva internal and external contractual key performance indicators (KPIs) which will include clinical outcomes, patient experience and utilisation metrics
You will work to overcome any remote working barriers to communication and team work
You will share patient and programme feedback with your Clinical Services Manager seeking to optimise the programme and help drive improvements in our Oviva technology platform (OCS)
You will have the opportunity to attend monthly clinical supervision sessions and training sessions in order to develop your skills whilst in role.
You will:
Deliver one-to-one or group coaching remotely via phone or using the Oviva app
Utilising expert diet and nutrition knowledge to support patients with a range of evidence based dietary approaches
Supporting patients remotely to make positive, practical changes to their diet and lifestyle
Utilising behavioural change, and motivational techniques, to support patients through the length of our programmes
Being accountable for collecting attendance, satisfaction and outcome data and work with Oviva to provide service evaluation and reporting
Maintaining accurate and up-to-date clinical records using Oviva’s secure electronic medical record system
Handling of sensitive data, approaching all areas of the role with great attention to detail
Completing Oviva training courses and keeping up to date with changes to programme processes
Adhering to all Information Governance and data protection regulations in the handling of patient data
Safely managing clinical risk incidents, adverse events and out of range readings according to Oviva protocol
Following the Safeguarding policy and protocol and Incident policy at all times.
Working in an autonomous manner, being responsible for appropriately managing clinical risk when it arises during clinical practice, while having recourse to professionals for guidance
Essential:
Experience in nutritional management of patients with weight management conditions or diabetes
Experience of managing a large patient caseload
Experience and ability to work autonomously and in a team
Strong ability to handle sensitive data and a great attention to detail
Evidence of maintaining CPD post-registration and continuing to meet AfN registration requirements
Excellent communication skills, as well as strong organisational and time management skills
Emotional resilience and the ability to thrive in challenging situations
Highly organised and flexible to adapt and keep up to date with a high paced environment
Competent user of IT equipment and software including an electronic patient record, as well as email, word processing and data input
The ability to be flexible and adapt to the needs of the contracts and the wider team
Desirable:
Experience delivering on NDPP, DSE or T2WM services
Experience of managing safeguarding scenarios
BCT and/or Motivational Interviewing training and experience of using this learning in practice
